Key Nuances & Insights

01The public grade codes are fragments, not a band manual

Live requisitions confirm A2, S2 and S4 support codes, P1 through P5 professional codes and M1 through M5 management codes, and a Shanghai supply chain posting maps M5 to Associate Director. That is the whole of the first-party evidence. Boston Scientific has never published a universal grade-to-title matrix, so the ladder shown here is a reconstruction and every pay figure attached to it is either an employer-posted range or a model built on one.

02The ESPP is the real employee equity programme

The purchase plan is broad, well specified and available in designated countries outside the United States. The 2011 long-term incentive plan permits grants to officers, directors, employees and consultants, but no public evidence establishes a minimum grade or a recurring grant below senior leadership. That is why this report carries annual equity at zero for A2 through P3 and M1 through M3 rather than assuming a token grant.

03Executive pay is overwhelmingly equity-linked and currently under water

About three quarters of the chief executive's 2025 reported compensation was stock and option value against 6.1 percent salary. The 90,915 options granted in February 2025 at a $106.14 strike had no intrinsic value at the $49.86 report-date price, so the accounting value in the proxy and the realisable value at the report date are very different numbers.

04Bonus funding is unusually transparent, bonus targets are not

The company discloses the funding factor, the individual modifier, the metric thresholds and the 225 percent maximum, and it discloses that funding hit the 150 percent cap in both 2024 and 2025 with 7 percent and 5 percent differentiation pools. It discloses no target percentage for any employee below officer level, which is the single biggest gap in the whole picture.

05Local pay is function-sensitive, so site type matters more than country

The manufacturing plants in Ireland, Costa Rica, Malaysia and Puerto Rico should not be benchmarked with the same mix as a commercial office in Zurich or an R&D centre in Shanghai. Dorado in Puerto Rico is priced in dollars but sits at roughly 0.64 of the Marlborough factor, because it is an operations site rather than a corporate one.

07The lateral versus internal pay gap is plausible but unproven

Nothing in the public record supports a Boston Scientific-specific estimate of how much a new joiner is paid above an internally promoted peer at the same grade. The bundle deliberately declines to assert a premium without internal payroll data, and so does this report.

08The 2026 restructuring is the main forward risk to every figure here

A plan announced on 27 July 2026 expects $700 million to $800 million of pre-tax charges, $600 million to $700 million of cash outlays and about $500 million of gross annual savings by 2029, with some headcount reductions. Labour condition applications, filed petitions and USCIS decisions are different universes with different refresh dates, and forcing them into one figure would be misleading.

Research control

Against Abbott, Medtronic, Stryker and Intuitive Surgical, Boston Scientific's chief executive package sits second of five for the cited periods, 2.8 percent below Abbott and 2.5 percent above Medtronic. Its employee-facing position is harder to judge because none of the peer group publishes salary bands either, and its 348 to 1 pay ratio is driven as much by a manufacturing-heavy median employee population of about 59,000 people, 60 percent of them outside the United States, as by the size of the chief executive package.

Known gaps and diligence before relying on a cell

  1. 1Vice President, Senior Vice President, Executive Vice President and Chief Executive Officer are deliberately excluded from the priced band table and carried in the executive section instead. Their pay converges on a single global market, and including them would drag every offshore city factor toward parity at the top of the ladder and produce cells such as an India-based officer on a United States officer package. The band table therefore stops at Senior Director, and the base and total factors at the top of the ladder are calibrated to that band.
